About Ontario Health Teams

The Ontario Health Teams (OHTs) are an initiative of the Ontario Ministry of Health. This new way of organizing and delivering care and services aims to facilitate their access as well as to improve the coordination of interactions between patients, families and providers in their communities.

An OHT includes hospitals, medical clinics, physicians, nurse practitioners, physiotherapists and many other healthcare professionals. Homecare, community and social service organizations are also part of an Ontario Health Team. All these partners aim to work together, as one large coordinated team within a region. To find out more about this new system, visit Improving Healthcare in Ontario.

The Archipel Health Team was born in 2023 from the merger of the Ottawa East OHT and the Prescott-Russell team in development.

Ottawa East includes the Vanier Ward, Lowertown and Orléans. Prescott-Russell includes eight municipalities: Clarence-Rockland, Hawkesbury, Casselman, Alfred and Plantagenet, Champlain, Hawkesbury East, The Nation and Russell.
