Lung Health
The Lung Health Program aims to provide a continuum of care for people with chronic obstructive pulmonary disease (COPD). The goal is to improve the quality of life of these individuals by helping them manage this chronic disease.
Mental Health and Addictions
The goal of this project is to plan access to mental health and addiction services (MHAS) taking into account the diverse needs of different communities, including Indigenous peoples, racialized communities, and francophones.
Best Practice Spotlight Organization (BPPO)
This project aims to collaborate with our partners to implement the best practices of the Registered Nurses' Association of Ontario (RNAO) program with a view to providing a continuum of care.
Patients Before Paperwork
This initiative supports the implementation of innovative digital solutions to make it easier for professionals to connect people to care. The goal is to adopt digital health tools that modernize clinical pathways and transform information sharing within the health care system.
Online appointment booking
This initiative supports the adoption of online appointment booking (OAB) solutions enabling patients and caregivers to book in-person or virtual appointments through self-serve online systems available 24/7.
