About Ontario Health Teams
The Ontario Health Teams (OHTs) are an initiative of the Ontario Ministry of Health. This new way of organizing and delivering care and services aims to facilitate their access as well as to improve the coordination of interactions between patients, families and providers in their communities.
An OHT includes hospitals, medical clinics, physicians, nurse practitioners, physiotherapists and many other healthcare professionals. Homecare, community and social service organizations are also part of an Ontario Health Team. All these partners aim to work together, as one large coordinated team within a region.
